The CCT provides
a "core and cluster service" for people requiring a greater
degree of support while maintaining a high level of independence. This
service is essentially a group of flats located very closely to an office
that is staffed 24 hours a day (ie: the flats are "clustered"
around the office -- the "core", hence "core and cluster").
As people livinig in this service become more independent they will move on
to more independent housing options (eg: they may chose to move on to
our "supported living"
or "independent living"
service)
People who may benefit from such an arrangement can be broken down into
three groups...
1. People
who have left home, hospital or any other long-term living arrangement,
who would like to learn the skills necessary to live independently.

2. People
who would like to develop their inter-personal skills. A lack of inter-personal
skills will frequently result in difficulties for this person within
the community.
3. People
who have had forensic involvement in the past. The cluster housing option
offers the person we are supporting the opportunity to receive behavioural support and
intervention when required. While living in this service this person
can learn how to live in the community, while living in a safe and supervised
environment.
Core and Cluster "Diminishing Support" Service
This service is similar to the Core and Cluster Service in that people
live in individual flats with visits from support staff. It is different
to the standard "Core and Cluster" model in that the people we support generally
require a small degree of support and a high level of independence. Over
time, the input from support staff will diminish, and rather than people moving on to more independent housing, they will remain
where they are and the staff will withdraw.
